Conditions of admission and pre-admission of patients

Welcome to an establishment of the Groupe de Santé CLINIFUTUR

The medical and administrative teams of the Groupe de Santé CLINIFUTUR take care of you to complete the entry formalities as well as possible.
Then, they accompany you until the end of your medical course. The different categories of staff are recognizable by their clothing and all staff are identified.

During your visit to the specialist doctor, the latter agreed with you on the date of your hospitalization and gave you an admission form.

There are several types of hospitalization

The durations and conditions differ according to the type of treatment to be received:

  • Complete hospitalization
  • Weekday hospitalization
  • Day hospitalization (or ambulatory)

Medical consultations before hospitalization: specialist consultation and anesthesia

The pre-anaesthetic consultation is an obligation before any intervention requiring anesthesia. On this occasion, you can ask the anesthetist all the questions that concern you. He will be able to inform you and reassure you about this medical procedure.

Pre-admission

We invite you to go to the reception of the clinic to make your pre-admission. The hostess will ask you to present a number of documents proving your status as insured:

  • Carte vitale and up-to-date certificate
  • Up-to-date health insurance card
  • Admission slip issued by the doctor
  • Identity document or residence permit
  • Proof of address less than 3 months old

By compiling your file, the hostess will be able to tell you the level of coverage by the health insurance plan and the costs that will be your responsibility.

If you are insured by social security: either the accommodation costs are covered at 80% (the co-payment of 20% is your responsibility or that of your mutual insurance company during the first 30 days of your hospitalization), – or the costs of stay are 100% covered, long-term illness, hospitalization from the 31st day, postoperative within 20 days.

If you are a foreign national or not insured by social security: you are liable for all accommodation costs.

The daily hospital fee

In accordance with the regulations in force, you will have to pay the daily hospital fee. Most mutuals cover this hospital package.
Exempt from it are workers injured or suffering from an occupational disease, beneficiaries of maternity insurance, invalids and victims of war.

Fee supplements

The practitioners providing your care may have opted for the conventional fee-free sector. In this case, they are authorized to practice excess fees which will remain at your expense. The information must be clearly communicated to you by the practitioner before your admission.

Hospital admission

All of our staff are bound by discretion and professional secrecy. You can ask that no indication be given of your presence in the establishment. To do this, make a simple request to the admissions office.

Thanks to the formalities completed before your hospitalization (pre-admission), the admission procedure is simplified. Your file is completed at the admissions department on the day of your arrival. A deposit check may be requested as well as an identity document.

To complete the medical file, you will need to bring the following items:

  • Elements not present during pre-admission
  • Results of radiological and biological examinations requested by the surgeon and/or the anesthesiologist
  • Blood group card
  • Current treatment orders
  • If minor or adult incapacitated patient: authorization to operate and specific documents (identity document and presence of the legal representative at all stages, family record book or copy of the guardianship or divorce judgment)
  • Don't forget your personal belongings (towels, toiletries, nightwear and slippers). It is best to leave your mobile phone, jewellery, cash and bank card and other valuables at home.

Pre-Admission / Admission

A multidisciplinary team of professionals subject to professional secrecy is at your disposal, welcomes you, and accompanies you during your care during the medical course. The different categories of staff are recognizable by their clothing and their identification.

Optional hotel services

For your comfort we can provide you with various hotel services

  • the single room (assignment according to availability) for accompanying persons in order to allow them to stay with the hospitalized person (sleeping, meals)
  • television
  • the phone
  • wifi
  • high-end meals
  • etc…

We will ask you to respect the operating rules of the establishment which you will find in the welcome booklet.

Your discharge from hospital

  • The date of your discharge is decided by the Doctor
  • Tours take place in the morning

If you are a minor patient, any outing can only be done in the presence of a parent or guardian.

The doctor will give you the various exit letters and prescriptions. Before your departure, go to the exit office, in the reception hall, to complete all the administrative formalities. A situation report specifying the duration of your hospitalization, intended for your employer and your health insurance fund, may be issued to you.

The invoices for your stay will be sent to you by post. You will have to pay the costs not covered by social security or mutual insurance: telephone, accompanying costs, etc. and you can recover the deposit for the remote control.

Any unauthorized outing, carried out at your sole request, requires that you sign a discharge releasing the establishment and the doctor from all liability.

On medical prescription and under certain conditions, an ambulance may be made available to you for your return.

Remember to complete and submit the satisfaction questionnaire when you leave. Your answers and comments are important to improve your stay.
